DEVON GASTON

Dr. Devon Gaston is a board-certified Functional Neurologist and Founder of Passport 2 Health. She has spent the past decade helping patients overcome chronic pain, balance disorders, and neurological conditions through advanced, integrative care. Based in El Dorado Hills, Dr. Gaston is a trusted resource for local families seeking relief from complex neurological and autoimmune conditions.

Through Passport 2 Health, she blends cutting-edge, objective diagnostics in functional neurology to deliver personalized therapy. Dr. Gaston is also an established medicolegal expert witness, specializing in traumatic brain injury (TBI) diagnosis and rehabilitation. Her expertise is strengthened by her diagnosis and subsequent recovery from a stroke with chronic intermittent vertigo, fueling her passion to advocate for patients in both clinical and legal settings.

Outside the clinic, “Dr. D” is a dedicated supporter of the triathlon community and enjoys exploring the trails around Folsom Lake, wine tasting in Amador, and playing the piano.

JACLYN “KAT” COOK LMT

Kat Cook is one of Dr. Gaston’s first functional neurology patients. In 2016, Kat had been misdiagnosed with narcolepsy after having two brain stem strokes in utero. She went from bedridden to becoming a licensed massage therapist after working with Dr. Gaston. “Functional neurology gave me my life back,” she shares.  

Kat is also Erchonia laser certified and incorporates the laser with her massage, Graston fascial releases, deep tissue, and hot stone massage.

Coach Cody Forman Passport 2 Health

CODY FORMAN CPT

Coach Cody is a NASM-certified Personal Trainer, Corrective Exercise Specialist, and professional triathlete. Cody has enjoyed working with some of the country's best Physical Therapists, Strength and Conditioning Coaches, and Endurance Coaches. From Jon Hodges to Bobby McGee, Cody's practical experience is well adapted for just about everyone's particular circumstances.

He graduated from the University of Nevada, Reno, with a Bachelor of Science in Biology and shadowed as a Physical Therapy Assistant in the Sports Medicine Department.

As a coach, he aims to promote fitness and well-being in a safe, fun, and engaging environment. He wants to encourage all his patients and clients to gain autonomy and feel better about themselves and their movements.

Carol Kendrick Passport 2 Health Team

CAROL KENDRICK

Patient Care Coordinator

Carol is a seasoned entrepreneur with a background in medical logistics, having been trained by the U.S. Army at Fort Sam Houston in San Antonio, Texas. She later served with the 7th Medical Division at Fort Ord, California. In 2020, she returned to the medical field to reconnect with her passion for helping others.

Carol is Erchonia® laser certified and trained to administer Class II medical laser therapy both in-office and through at-home rental programs.
